About NEPSHQ DocGen
NEPSHQ DocGen (New England Public Safety Headquarters DocGen) is a web-based documentation and report-writing platform designed to make structured report creation faster, easier, and more consistent.
DocGen was created by public safety professionals with more than 20 years of real-world experience in emergency medical services, fire service operations, emergency communications, public safety administration, training, and documentation. The platform was developed around a simple idea: report writing should support the provider, not slow them down.
Built for Easier Report Writing
Public safety documentation can be repetitive, time-consuming, and easy to overlook when crews are busy. DocGen provides structured forms, narrative builders, calculators, checklists, reusable templates, and configurable documentation tools that help users organize information and generate clear, consistent text.
Depending on the template being used, DocGen can help users:
- Build structured EMS, fire, public safety, and administrative narratives.
- Use detailed assessment prompts to reduce commonly missed documentation elements.
- Generate DCHART, SOAP, CHATT, CHATT-O, DACHARTE, chronological, and other narrative formats.
- Use clinical and operational calculators.
- Complete assessment, readiness, QA, equipment, operational, and documentation checklists.
- Create organization-specific forms and templates.
- Customize existing templates without having to build every tool from scratch.
- Generate text that can be reviewed, edited, copied, printed, or transferred into the appropriate reporting system.
DocGen is intended to function as a documentation aid. It does not replace professional judgment, agency policy, medical direction, applicable protocols, required documentation standards, or the user's responsibility to ensure that the final report is complete and accurate.
Review Every Generated Report
Generated text should always be reviewed before it is submitted or placed into an official record. Automated or template-generated language may require editing to accurately reflect the specific circumstances of an incident, assessment, treatment, response, or patient encounter.
The person using DocGen remains responsible for verifying the accuracy, completeness, appropriateness, and final content of any generated documentation.
Important Privacy & HIPAA Notice
DO NOT ENTER PATIENT INFORMATION INTO DOCGEN.
Do not enter protected health information (PHI), personally identifiable patient information, medical record numbers, dates of birth, Social Security numbers, patient addresses, telephone numbers, insurance information, or any other information that could identify a patient.
DocGen is intended to help create generic documentation language and templates. It is not intended to serve as an electronic patient care reporting system, electronic medical record, or storage location for protected health information.
New England Public Safety Headquarters (NEPSHQ) and the operators of DocGen are not responsible for HIPAA violations, privacy breaches, unauthorized disclosures, or other consequences resulting from a user's decision to enter, copy, upload, store, transmit, or otherwise place patient-identifying information into DocGen.
Users and organizations are responsible for complying with HIPAA, applicable state privacy laws, agency policies, contractual obligations, and all other requirements governing protected or confidential information.
Privacy by Design
DocGen is designed so that answers entered into a normal template are used to generate the requested output and are not intentionally stored as patient records by default. The platform is designed around reusable templates and generated documentation rather than maintaining a patient database.
However, users should never rely on that design as permission to enter PHI. Information may still be exposed through browsers, devices, screenshots, copied text, server logs, browser extensions, network systems, backups, or other technologies outside the intended DocGen workflow. For that reason, the safest rule is simple: do not enter patient-identifying information into DocGen.
Clinical & Operational Disclaimer
Clinical calculators, checklists, assessment prompts, narratives, and other tools provided through DocGen are informational and documentation-support resources. They are not a substitute for current medical protocols, medical control, formal clinical education, agency policies, manufacturer instructions, or professional judgment.
Protocols, scoring systems, clinical recommendations, laws, standards, and accepted practices can change. Users should verify information against current authoritative sources before relying on it for patient care, operational decision-making, or official documentation.
Customizable by Design
DocGen is built around reusable system templates that can be expanded over time. Administrators can maintain system-wide templates, while users can work with forms and documentation tools appropriate to their needs. The goal is to create a growing library of practical documentation resources without forcing every organization or provider into a single reporting style.
New England Public Safety Headquarters
NEPSHQ develops practical tools and resources intended to support public safety professionals and organizations. DocGen reflects that mission by focusing on usability, consistency, adaptability, and the realities of documentation in the field.
DocGen helps you write the report. You remain responsible for the report.
